Chapter 18: Quality Assurance

The Swiss cheese model of accidents considers various error prevention barriers to be like slices of Swiss cheese. The barriers (slices of cheese) are stacked in place to prevent errors, but none of these checks functions perfectly, which is represented by the holes in the cheese. Quality assurance (QA) measures are analogous to these Swiss cheese slices, and the goal of this chapter is to present best practices for QA.
